
Death-benefit
coverage is automatic
If you retired from
state service after April 1, 1970, your surviving
beneficiary is entitled to a $3,000 benefit upon your
death.
You dont have to enroll in the Survivor Benefit
Program. You are automatically covered when you have at
least 10 years of full-time state service within the 15
years immediately prior to retirement. You must have
retired directly from state service.
If you selected the Single Life Allowance option, you
must name a beneficiary using Form R56355. On all other
choices, a beneficiary is already named and there is no
need to choose a beneficiary, except if you wish to
change the beneficiary.
To change your beneficiary, write to the Retirement

The death of a pension recipient must be reported
immediately, so payments may be stopped.
